Not able to create reports, related module fields are not displaying

It seems you are facing some PHP errors which break the Report's js execution.

Open the developer tools in your browser (F12 for Chrome and Firefox), go to console tab. Hopefully you will see some PHP errors in the console tab.

The appropriate fix depends on the PHP errors.

Follow these steps and let us know the errors so we can guide you on fix.

Yes I am getting some errors in console and I am keeping screenshot related to those errors. Please guide me to solve this issue.

Please, let us know also the errored request at the network tab. You should be able to see the request by clicking on the link at "modules_def_en_us_93...". On accessing the buggy request open the sub tab "Preview" at the Network tab and let us know the content.
