When did the salutations get added to the contacts module

The dawn of time I think...  We don't use it.

2004 when SugarCRM was started. This is configurable. 

Right - it's always been there, but now it is part of the top. For example title always been there but never part of the top as a default for the name. See screen shot

Also, its not configurable in studio  - you can not remove it as it's now part of the name.

No need to remove, it only displays if a salutation is selected, if not it disappears on save.

The Salutation displays depending of the users profile settings on Advanced Tab -> Locale Settings

If the users picks a name display format having the salutation included, the field will be displayed.

I recommend to set the included salutation as default for this

Admin -> System -> Locale

as users not having the salutation displayed can't fill it out and produce inaccurate records :-)
