Can't convert a lead

Hi,

I'm using 7.6.1. Pro.

When trying to convert a lead, if I try to create both a new account and contact, then the 'convert' button remains inactive. I've tried to ensure all the required fields of these two records are complete.

If I associate both an existing contact and account, then I can 'convert' the lead.

Any thoughts on the best way to trouble-shoot this?
