After playing with this for a few minutes, I think I've figured out how this works. It looks like the attachments are actually stored via a relationship to the Notes module.
So, in the Filters step, click on the relationship Emails:Notes and select the Attachment field, with the condition Is Not Empty.
You can show the attachment file name in your report by adding the Attachment field as a Display Column in the same way.
(Note that I've assumed you are using Sugar Pro or Ent, not CE)
