I followed this article

was able to successfully achieved what I want but my customer wants it not for record view. It needs to be on "Create Case" Screen because that data only requires on time of case creation
.
I tried everything without any luck , did not find anyway to add a dashlet on "Create View".
Please help , very very urgent.
